Q1. How does the Personalized Care Practice work?
A. Our Personalized Care Practice is a subscriber-based program, dedicated to building strong doctor-patient relationships and focusing on individualized care. Subscribers pay an annual fee. Subscription is limited to 400 patients, in order to ensure each patient receives the intensive and individualized care that is our goal. Those who subscribe will have direct access to Dr. Maldonado. During office hours, staff answers the phones and schedule appointments for that day or the next day. After business hours, all calls go directly to Dr. Maldonado's personal cell phone, thus eliminating the often frustrating experience of having to deal with an answering service. When patients arrive for appointments, they are seen promptly and may take as long as they need to have all their questions and concerns addressed.
In addition, each subscriber receives a complimentary annual wellness evaluation and email and website availability. Any needed blood draws or urine samples are taken in-office, so that subscribers don't have to wait at outside labs.
Dr. Maldonado personally calls each subscriber to discuss the results from such tests.
Q2. What if I’m hospitalized?
A. Dr. Maldonado personally coordinates hospital care for all subscribers, visiting patients and arranging for sub- specialist consultations when necessary. He can also expedite Emergency Room visits when necessary and will discuss cases with local or out-of-town family members. Dr. Maldonado has admission privileges at Naples Community Hospital and North Collier Hospital.
Q3. What if I’m too sick to come to the office?
A. As has always been Dr. Maldonado's practice, if you are too sick to come to the office, he will continue to make home visits when necessary and appropriate. When you call, he will discuss your condition with you to determine whether you would be better served by an office or home visit.
Q4. Is the Personalized Care Practice a substitute for health insurance?
A. Because it does not cover the cost of actual medical care (other than the annual wellness evaluation), the Personalized Care Practice is not a substitute for health insurance. Subscribers need to keep their health insurance and Medicare coverage to cover such costs as office visits, subspecialty physicians, lab tests, and hospitalizations.
Q5. What about preventive health measures?
A. Through the Personalized Care Practice, Dr. Maldonado will have the time to get to know you and your particular health concerns. This allows him to make specific health-promoting recommendations, tailored to your personal condition, keeping the focus clearly on your health. You will also find regular updates on his website, reporting the latest advisories and recommendations.
Q6. Will my insurance cover the membership fee?
A. The annual membership fee is not covered by insurance or Medicare. The fees are for services and amenities that are not covered by Medicare or insurance.
